JOB DESCRIPTION

Home Care PT

Coverage area: City of Milwaukee and Wauwatosa

Candidate MUST HAVE:
- 2 years PT Home Health experience with Adult/Geri patients.
- active PT license (WI) and BLS (AHA)
- valid Drivers License and auto insurance

Technical Skills required: candidate must be able to assist with ADLs and adaptive equipment. Able to effectively collaborate with other specialties (RN, OT, Speech, etc.). Oasis training is needed for this position.
