Nationwide Therapy Group is seeking a LPN / LVN Home Health for a travel job in Lafayette, Indiana.

Position Overview

Setting: Home Health - patient visits in their homes
Documentation: Must be proficient with OASIS
Schedule: Monday through Friday
First-Time Travelers Accepted: Yes
Floating: Yes - Required to float between regional offices based on need
Local Candidates: Not accepted (Indiana residents are considered local)

Requirements

Experience: Minimum 18 months of LPN experience (Home Health strongly preferred)
Certifications: BCLS (Required)
License: IN or Compact License in hand OR must have started licensing process if assignment begins 4+ weeks out
COVID Vaccine: Not required
Flu Vaccine: Unknown
