We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in a variety of settings, including home community care and geriatric facilities. As an LPN, you will work under the supervision of registered nurses and physicians, assisting in the delivery of patient care while ensuring adherence to safety and health regulations.

Responsibilities

Administer medications and treatments as prescribed by healthcare providers.
Monitor patient vital signs and report any significant changes to the supervising nurse.
Assist with daily living activities such as bathing, dressing, and meal preparation.
Provide catheter care and assist in catheterization procedures when necessary.
Maintain accurate patient records using eClinicalWorks software.
Participate in sterile processing of medical instruments and equipment.
Support patients with their nutritional needs, ensuring proper aliment intake.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Lift and transfer patients safely, adhering to proper body mechanics.

Skills

Proficient in using eClinicalWorks for electronic health records management.
Knowledgeable in catheter care and catheterization techniques.
Familiarity with sterile processing protocols within a healthcare setting.
Strong understanding of geriatric care principles and practices.
Ability to perform laboratory tests as required by the healthcare facility.
Excellent communication skills to effectively interact with patients, families, and healthcare team members.
